Clery Act Campus Security Authorities CU-Colorado Springs (UCCS) Crime Statistics Reporting Form

Please use this form to report Clery Act crimes. This data will be included in UCCS's Annual Security Report, as required by the federal Clery Act. If you've witnessed or learned of a Clery Act crime, immediately call UCCS Police Dispatch at 719-255-3111, then fill out this form. Submit one form per crime/incident.

If a serious crime that may cause an imminent or ongoing threat to the campus community is reported to you, immediately contact UCCS Police Dispatch at 719-255-3111. If there is any question about whether an ongoing threat exists, immediately contact UCCS. Federal law requires universities to notify their affiliates about any crimes that pose an ongoing threat to the community. Campus Security Authorities are obligated by law to report these crimes immediately.
